Douglas County School Registration Requirements

Information for Parents

To better serve the families in our community, the Douglas County School System is excited to announce the opening of the new "STUDENT REGISTRATION CENTER" for the 2006-2007 school year. All students who are new to our school system, including Kindergarten, will need to register at the Center.  In county transfers will still be maintained from school to school.

NO SCHOOLS WILL ENROLL


Parents should obtain the following documents, that are listed below, and bring them to the Registration Center.  The Registration Center is located at 4841 Highway 5 in Douglasville.

From Annette Winn, go west on I-20 to Exit 34, Highway 5, turn left at the traffic light.  Drive for 5 miles until you see the Bill Arp Ball Park.  The Registration Center is across from the Dollar General.  Follow the sign and drive around back.

Scheduled appointments are taken by calling 770-651-2050 to set one up.  Students do not have to be present to enroll.    Hours for registration are Monday - Friday, 8:00 A.M. until 11:45  1:00 until 4:30 P.M. (closed for lunch 12-1:00 p.m.).

As a reminder, so that enrolling your student/students will be minimum, Parents/Guardians should obtain the following  documents and bring them when enrolling a student into the Douglas County School System.  A child may be denied access from registering without all the required papers.  If the school year is in effect and you have not withdrawn your student from the previous school, you must do so before entering our school system.

DOCUMENTATION REQUIRED AT REGISTRATION
*We need these items before you will be put on a class roll.

*Proof of Age:  (such as an original/certified birth certificate with certificate number). Children who live in Douglas County and who will be five years old (GA. law) on or before September 1, 2006 are eligible to enroll in kindergarten.
*Certificate of Immunization:  Georgia form 3231 must be used for registration.
*Certificates of Ear, Eye, and Dental Examination: Georgia form 3300 is needed.
*Social Security Card Number:  (may be waived if a parent signs a statement objecting to the requirement O.C.G.A. 20-2-150d).
Vehicle tag receipt: shows resident in Douglas County or natorized statement on file
*2 Proofs of Residence:  required are your vehicle tag receipt, plus documents showing your street number and name, city and state (lease agreement, property tax bill, mortgage statement) and a gas, power or water bill.
If you do not own and are living with someone they must sign a 'multi-residence' form and they must provide the proof of residence.
Withdrawal papers from previous school
Custodial Orders of Guardianship Letters:  are needed when a child does not reside with the natural parents.  The custodial parent or guardian must present certified copies of custodial orders or guardianship letters at the time of registration.
International Families must show proof of child's birth such as a passport pink or green card, baptismal certificate with a birth date, Federal I-94 card with birth date or copies of permanent records that verify the child's age from another U.S.A. school district.
Undocumented Residents should be referred to the Georgia Mutual Assistance Association Consortium.  It is recommended that international families bring someone who speaks English to registration.

 You may see your personal physician or go to one of the health departments listed below for your immunization forms 3231 and 3300.
